Esthetics with Bonded Porcelain Restorations- A Distinctive Approach to Nature.

THE MOST EXCITING DEVELOPMENTS IN DENTISTRY HAS EMERGED

WITHIN THE PAST DECADE. ORAL IMPLANT DENTISTRY, GUIDED TISSUE

REGENERATION, IMPROVEMENT OF CAD/ CAM TECHNOLOGIES, AND

ADHESIVE RESTORATIVE DENTISTRY ARE GROWTH AREAS. HOWEVER,

BONDED PORCEAIN RESTORATIONS OFFER A RESTORATIVE SOLUTION

THAT BALANCES THE FUNCTIONAL AND AESTHETIC NEEDS OF THE

ANTERIOR DENTITION. PORCELAIN’S OPTIMAL STIFFNESS, ITS IDEAL

SURFACE CHARACTERISTICS, AND THE BIO - MECHANICAL STRENGTH

ACHIEVED THROUGH HIGH - PERFORMANCE BONDING ENABLE THE

CROWN OF THE TOOTH AS A WHOLE TO SUPPORT INCISAL OR

MASTICATORY FUNCTION. BY THE SAME TOKEN, THE OPTICAL

EFFECTS INHERENT IN THE TOOTH AND THE LIFELIKE FEATURES OF

THE PORCELAIN MAKES THIS RESTORATIVE APPROACH ULTIMATE IN AESTHETICS

Lecture:

Requirements and Treatment Protocols for the Ultimate Outcome of Red and White Esthetics in Implantology, from socket preservation, augmentation and maintenance of bone and soft tissue in a long term basis.

The high success rate in implant osseointegration with long time results (more than 10 years) of about 98 % is evidence based and documented in plenty of scientific papers worldwide.

More and more in the focus of interest in the last couple of years is the predictable aesthetic outcome with the balance of hard and soft tissue.

The wishes and demands of our patients are the accelerated implant therapy with a short treatment period, minimum of operation trauma, cost effective and predictable implant concept and a high aesthetic outcome of the ceramic frame and periimplantological soft tissue with a physiological papilla formation for the red aesthetics.

For that reason we need implants and treatment protocols which preserve, create and maintain the bone in the aesthetic osseointegration.

In the first part of the lecture we summarize the requirements for the aesthetic outcome respecting the biological width, the implant macro and micro design, the implant position and angulations, the dynamic osseointegration and bone training with temporary abutments and prosthetic solutions to achieve and maintain the hard tissue.

In the second part of the lecture we look at clinical cases from single tooth replacement to multi teeth defect cases with 8 to 10 implants, especially in cases of limited bone amount .

Treatment planning in a prosthetic driven way of understanding by using 3 D data and final restorations in all ceramic for the high esthetic outcome round up the lecture.

Into the future - advances in dental technology and materials

The application of digital technology in dentistry has developed from an inconvenient truth to reality. The focus is on patient’s communication, digital treatment planning for surgical and prosthetic options, digital impressions and the state of the art and limitations of current CAD/CAM systems. New materials are aiming at optimizing biocompatibility and longevity of dental restorations.

Mr. Ernst Hegenbarth MDT owns a laboratory focussing on functional and esthetic rehabilitations with attached Teaching Institute for post graduate education in Germany. Active member of the “European Academy of Esthetic Dentistry”, Associate member of the “American Academy of Esthetic Dentistry” and the scientific board of several dental journals. Author of numerous publications on function and occlusion, dental ceramics, esthetics, Dental CAD/CAM-Technology and implantology. Lecturing extensively worldwide since 28 years.

"Esthetic Approach": How to combine predictability and repeatability using bonded ceramics

Because of new patient demands, laminate veneer rehabilitation is becoming common and popular. This therapeutic method needs a multidisciplinary approach (periodontology, orthodontic treatment) to achieve the final result.

For most dentists laminates are very exiting but a only a few of them are using it. This phenomenon can be explained with a list of black points which deter the dentist : the choice of the material, type of esthetic project and how to realize it, prep approach and the concept evolution and adhesive procedure.

All these point will be discussed tin order to achieve a simple and predictable result.

In order to allow the patient to have a preview of the final result before any treatments, we will proceed in a different and original way.

In deed, after a complete clinical and paraclinical exam, we do a precise diagnosis that allows the creation of the esthetic project through a wax up.

This will be transferred in the mouth (mock up) using a silicon key and a temporary resin therefore the patient can visualize the design modification of his smile.

Then we can start a pre-prothetic treatment (muccogingival step) and at last the teeth reduction will be made using the silicon guide .

Laminate veneer production requires meticulous and artistic qualities on behalf of the technician.

To obtain predictable results, bonding techniques will be organized according to a precise and strict methodology.

Bonding techniques and modern ceramics associated with a clinical methodology are the basis of the new esthetic dentistry ; Biomimetic approach (Magne) is the aim of our practice.

Lecture:

Hollywood and Natural Esthetic Dentistry:

Understanding the differences and how to be successful with both.

Cosmetic and esthetic dentistry has been very popular for several decades and is now a world wide phenomena. Because of the internet and popular makeover programs, patients are more educated and have higher expectations about the type of smile they want to attain. Beauty is in the eye of the beholder. Whether the patient chooses a Hollywood smile or decides to go with a natural esthetic smile, the modern dentist needs to know how to create both styles. This presentation focuses on techniques, materials and patient management skills to achieve predictable success with both Hollywood and natural dentistry.

Learning Objectives:

1 Understanding the differences in creating a Hollywood smile vs. a natural smile.

2 How to make bleached esthetic restorations appear natural and not look like “white chicket teeth”.

3 How to create predictable and consistent esthetic success with progressive dental materials and techniques.
